The Yale-New Haven Hospital Center for Outcomes Research and Evaluation (CORE) is a leading national outcomes research center working on select projects designed to assess healthcare quality and evaluate clinical decision making and comparative effectiveness of specific healthcare interventions. Our mission is to improve health and health care through targeted research and evaluation projects that combine scholarship and service to have a positive impact on policy and people’s lives.
We have developed the publicly reported readmission measures for the Centers for Medicare and Medicare Services (CMS), which can be found on their Hospital Compare website. In the course of developing those measures, we produced statistical models based on medical record data which can predict the risk that a patient hospitalized with a heart attack, heart failure or pneumonia will be re-hospitalized within 30 days of leaving the hospital. These models are published in the medical literature. These are the same models that were disseminated by the American College of Cardiology and the Institute for Healthcare Improvement Hospital to Home (H2H) Program, a national effort to reduce the risk of readmission.
This App provides a readmission risk calculator based on those published models. They are superior to many other models because they use detailed information about each person and were designed specifically to predict readmission risk.
This readmission score is the best estimate of a patients risk of readmission given the information provided. The risk for any individual could vary from this estimate as there may be other factors beyond what are in the model that may influence readmission risk.
